​Personal Data Protection Policy


Green Avenue Pte Ltd (“GA”) respects the privacy and confidentiality of the personal data of our residents, volunteers, visitors and others whom we interact with while providing our services. This Personal Data Protection Policy (“Policy”) inform the ways in which we collect, use and protect your personal information.


This Policy is based on the Singapore Personal Data Protection Act 2012 (“PDPA”) and all the associated regulations and guidelines as may from time to time be issued by the Personal Data Protection Commission (“PDPC”) of Singapore.  By providing your personal data to GA by you or through your Company or Group of companies, you agree to the terms of this Policy.

1. Collection on Personal Data
GA, in carrying out its business, may request you to provide personal data such as, but not limited to:
•    Personal details 
•    Personal contact information 
•    Medical/Health information 
•    Financial information 
•    Educational and professional qualifications 
•    Professional and work experience 
•    Photographs / Video Footages / Audio Recordings


2. Use of Personal Data Collected
We use the personal data for the purposes you have been notified of and consented to or which are not prohibited by applicable law. This may include but not limited to:
•    Identity verification
•    Registration of residents
•    Application of grants and financial aid
•    Provision of nursing and medical care
•    Referral to other service providers in relation to care
•    Government audits and applications for government schemes
•    Processing of billing for services
•    Processing of volunteer management and donations
•    Handling of queries and feedback 
•    Facilitate product and service development 
•    For safety and security purposes
•    Complying with any applicable laws and regulations in the industry that GA operates in
•    Transmitting to relevant third-party service providers, agents,  governmental authorities, in Singapore or overseas
•    Any other usage which GA may inform you of, from time to time, but for which GA will seek your separate consent


3. Disclosure of Personal Data
We may disclose your personal data to third-party service providers, agents, governmental authorities and other organisations in order to fulfil our services to you.  This may include but not limited to:
•    Clinics, hospitals, hospices, pharmacies
•    Medical practitioners
•    Social services agencies
•    Regulatory authorities and government agencies
•    Transport, courier services
•    Cloud service providers


4. Withdrawal of Consent
If you wish to withdraw your consent, you should give us reasonable advance notice by contacting us at dpo@greenavenue.com.sg.  Upon receipt of your written request to withdraw your consent, we would require a reasonable period of time for your request to be processed and for us to inform you of the consequences of us acceding to the request. 


5. Access and Correction of Personal Data
If you wish to access any personal data that you provide to us, please contact us at dpo@greenavenue.com.sg.  If we should require charging a reasonable fee to process your request, you will be informed of the fee before we process your request. 


We will respond to your request as soon as reasonably possible or within the number of days as stipulated in the PDPA.  If we are unable to provide you with any personal data or to make a correction requested by you, we shall inform you of the reasons why we are unable to do so (except where we are not required to do so under the PDPA).
 

6. Accuracy of Personal Data
We will keep your personal data as accurate, complete and up-to-date as possible, by taking reasonable precautions and verification checks to validate your personal data before using them.

 

7. Protection of Personal Data
We have implemented appropriate information security measures to protect the personal data you provide us against unauthorised access, use, disclosure, or similar risks. We will take reasonable and appropriate measures to maintain the confidentiality and integrity of your personal data and will only share your data with authorised persons on a ‘need-to-know’ basis.


8. Retention of Personal Data
Your personal data is only retained for as long as the purposes for which the data is collected or used continues, or where necessary, for our business and legal purposes. GA will thereafter delete or destroy the personal data or restrict access to data which can be associated with you.


9. Updates to Policy
We reserve the right to modify or change this Policy at any time and will place notice of any such amendments on our website or through other suitable methods.
